Services and Fees

Court Ordered Assessment - $150.00 Prepaid

Court Ordered Documentation - $125 Prepaid 

(We require 5 business days’ notice. Add $50 for rush service.)

Sliding Scale Self-Pay: Must be paid on the date of scheduled appointment. 

To qualify for sliding fee service the client or clients must be unemployed, have a state funded insurance not contracted with Spade’s Counseling Agency or currently not earning a gross family income more than $60,000.00. 

Payment

 Cash, checks, and credit/debit cards are accepted for payment. 

Insurance

 Services may be covered in full or in part by your health insurance or employee benefit plan. Please check your coverage carefully by contacting your health insurance carrier and asking the following questions:

·     Do I have mental health insurance benefits?

·     What is my deductible and has it been met?

·     How many sessions per year does my health insurance cover?

·     What is the coverage amount per therapy session?

·     Is approval required from my primary care physician? 

·     If the therapist is not in the network, what will the insurance cover? 

It is your responsibility to pay for what insurance does not cover at each session on the day of the appointment, i.e. copays, self-pays, late cancels, no call/ no show fees.

BE AWARE: If we are not in your insurance network that does not mean that insurance does not pay for the sessions. It usually means that you will pay a higher deductible and copay. Check with your insurance company to determine the out-of-network rate.

Accepted insurances: Blue Cross Blue Shield, Aetna, Molina, Forward Health (Badgercare), United Behavioral Health (UBH), United Medical Resources (UMR), WEA Trust, Humana, and United Health Care.
